Steam gets into every gap a normal shower can ignore. The walls, ceiling and bench all need a continuous waterproof and vapor barrier, and the ceiling is usually sloped slightly so condensation runs to a wall instead of dripping on your head.
The enclosure has to close. That means glass that runs all the way up, or a transom, and a door with seals. A standard shower door with a gap above it will not hold steam.
The steam generator sits outside the shower, often in a vanity, closet or attic space nearby, and needs a dedicated electrical circuit plus a water supply. We plan that location during design so it stays serviceable.
On Musket Rim the homeowner asked for a high-end shower, and we built a hand-crafted tile shower and steam room with a bench, a niche shelf, frameless shower glass, new flooring, and the associated plumbing and electrical work.
On Cielo Azul the master shower got steam sprayers as part of a full master bath suite that also added heated floors and a jacuzzi tub.
Bench or no bench, where the controls go, whether you want the steam head away from the seat, and which wall hides the generator. Changing any of these after the walls are waterproofed is expensive, so we settle them before demolition.
If you only want the feel of a spa shower without the sealed enclosure, a large walk-in shower with multiple heads may be the better fit and budget.

Sometimes. The shower has to be fully enclosed to the ceiling, and the walls need a vapor barrier behind the tile. If your current tile has no vapor barrier, the walls usually come down to the studs so they can be rebuilt correctly.
Outside the shower, within the distance the manufacturer allows, in a spot you can reach for service. A vanity cabinet, linen closet or attic space next to the bathroom are common choices.
